Monday through Friday 8 a.m. to 5 p.m.

Intake Process:

Materials required for intake include:
- psychosocial Assessment,
- proof of Axis 1 Mental health diagnosis,
- letter verifying homeless status,
- proof of income,
- proof of TB test,
- proof of medical physical,
- list of medications, and
- letter from prescribing doctor stating that the client can or cannot self-administer meds.

Items that can be obtained after intake:
- photo ID,
- Social Security Card, and
- insurance Card.

Program Fees:

An apartment lease is completed and signed by client at intake; rent is charged according to income (based on HUD standards) and is approximately 30% of client's income.

No application fee, deposit, or utilities fee
